ALL PARENTS INTERESTED IN VOLUNTEERING WILL NEED TO GET CLEARANCE FROM TUSD

In an effort to maintain safety for our students and to comply with TUSD requirements for booster clubs or organizations associated with South High, all parents who wish to volunteer for any band events will need to be cleared for the 2021-2022 school year. Activities include Uniform Fitting Day, chaperoning games and competitions, and driving equipment vehicles. This applies to any person over 18 who would like to volunteer for even just 1 event. 

What to submit?
The following 4 items are required:

1. Complete the TUSD form entitled Application and Agreement for Volunteer Services and Code of Conduct.
2. Submit a copy of your driver’s license.
3. Submit a TB Risk Assessment Form signed by your physician. A TB test should be performed if you respond “Yes” to any of the 5 risk factor questions. TB clearance is valid for 4 years, so you can submit a signed form from the previous year.
4. Submit a copy of proof of COVID-19 vaccination.
